Woman Goes for Leg Operation, Gets New Anus Instead

this is sure to have the clinic dole out millions of dollars to the poor woman...i understand that there are gonna be cases out there where the charts are going to be accidentally switched, but what i don't understand is so the surgeons were unable to identify the fact that the woman did not need a new anus

geez, surgeons are so careless these days wasn't there an article about a woman going in to get rid of wrinkles and instead she got breast implants?

That is horrifying. I don't know much about these kind of surgeries, but couldn't they tell they were operating on a perfectly good anus?
